Please be advised that Probate Court forms have been updated to reflect regulatory changes made pursuant to section 106 of the Probate Act.
The revised Probate Court Practice, Procedure and Forms Regulations include references to a Commissioner of Oaths in the list of officials who may take an affidavit proving the execution of a will.
Forms have been updated to reflect this change, as well as to correct typographical errors and for clarity and consistency.
The regulatory changes were made in April 2026.
The unofficial templates of the revised Probate Court forms are available here.
